6TH GRADE PARENTS: Immunization Requirement for students entering 7th Grade. In order to avoid long lines at orientation, current 6th grade families can bring a copy of their updated Immunization Record to the office prior to the end of the year.  7th grade students must be fully immunized or have a permanent or temporary medical exemption on file at Arroyo Vista. Per state law, 7th grade students cannot have personal beliefs exemptions.  We suggest you make plans prior to the end of the school year for your child to receive the necessary immunizations, or speak with your physician regarding permanent or temporary medical exemptions. Only a licensed Medical Doctor (MD) or Doctor of Osteopathic Medicine (DO) may provide a medical exemption. Included in the required immunizations is the Tetanus, Diphtheria, Pertussis booster required for 7th grade. The booster may have been given any time after the 7th birthday.

Please bring your child’s Immunization Record and/or specific medical exemptions from your physician, to your child’s school health office as proof of immunization. School staff will photocopy the Immunization Record and enter the information in the district student data base. Your child will not be able to attend classes in 7th grade without proof of immunization or medical exemption on file with us for all required immunizations. For more information regarding the immunization requirements for 7th grade you may visit http://www.shotsforschool.org/k-12 .
